Uyarılmış Vestibüler Myojenik Potansiyeller (VEMP) testleri periferik vestibüler
bozuklukların tanılanmasında kullanılmaktadır. VEMP yanıtlarının sadece
sternokleidomastoid ve oküler kaslardan değil diğer kranial kaslardan da
alınabileceği bilinmektedir. Masseter Uyarılmış Vestibüler Myojenik Potansiyel
(mVEMP) testinde ise kayıt masseter kasından alınırken yolağı trigeminal sinirin
motornöronları ile vestibüler nukleuslar arasındaki bağlantıyı içerir. Böylece,
mVEMP vestibülomasseterik refleksin yolağını değerlendirmemize yardımcı olur.
Bu çalışmanın amacı ‘’Olası Meniere’’ hastalığı grubunda değerlendirdiğimiz
‘’Rekürrent Vestibübülopati’’ hastalarında mVEMP bulgularının değerlendirilip
mVEMP’ in vestibüler değerlendirme test bataryasına eklenmesidir.
Kontrol grup herhangi bir vestibüler ve sistemik hastalığı olmaya 18- 40 yaş arası 20
sağlıklı bireyden (13 Kadın, 7 Erkek) , çalışma grubu ise periferik vestibülopati tanısı
almış 18- 40 yaş arası 20 bireyden (16 Kadın, 4 Erkek) oluşmaktadır. Çalışma grubu
hastaları seçilirken en az 2 tane 20 dakika ve daha uzun vestibüler atağı olup işitsel
semptomu olmayan hastalara cVEMP ve oVEMP testleri yapılmıştır. Testlerden en
az birinde asimetri oranı %40 ve üzerinde olan hastalar çalışma hastası olarak kabul
edilmiştir. Hastaların masseter kaslarına yerleştirilen yüzeyel elektrotlar yardımı ile
kayıtlar alınmıştır. Elde edilen dalgaların latansları ve asimetri oranları istatiksel
analizde kullanılmıştır
Normal grup ve hasta grubu arasında latans değerleri açısından anlamlı bir farklılık
gözlenmezken, asimetri oranlarında anlamlı bir farklılık gözlenmiştir. |

Vestibular evoked myogenic potential (VEMP) tests are used to diagnose peripheral
vestibular disorders. It is known that VEMP responses can be taken not only
Sternocleidomastoid (SCM) and ocular muscles but also other cranial muscles.
Masseter Evoked Vestibular Myogenic Potential (mVEMP) test pathways include the
connection between the motorneurons of the trigeminal nerve and vestibular nuclei
while the recording is taken from the masseter muscle. Thus, mVEMP helps us
evaluate the pathway of vestibulomasseteric reflex. The aim of study is to add
mVEMP to vestibular evaluation test battery after evaluating the mVEMP findings
in recurrentvetibulopathy patients.
The control group consisted of 20 healthy participants ( 13 female, 7 male) aged 18-
40 years with no vestibular and systemic disease, the study group consisted of 20
participants ( 16 female, 4 male) aged between 18- 40 years who were diagnosed
with peripheral vestibulopathy. In the study group, cVEMP and oVEMP tests were
performed in patients who had at least two 20 min and longer vestibular attacks and
no auditory symptoms. Patients with a asymmetry ratio of % 40 or more in at least
one of the tests were accepted as study patients. Recordings were taken with
superficial electrodes placed on the masseter muscles of the patients. Latency and
asymmetry ratios of the obtained waves are used in statistical analysis.
There was no significant difference between the normal group and the study group in
terms of latency values, but a significant difference was observed in asymmetry
ratios. |
